Terms & Conditions

Please read every section below carefully. This agreement outlines what to expect from your Smile Flex Plan — from enrollment through your final set of aligners.

Before you begin: This agreement covers your enrollment, billing, and treatment terms for the Pay-As-You-Go Aligner Program. By signing below, you confirm that you’ve read and understood each section.

Program Description

The Smile Flex Plan is a month-to-month aligner program designed for minor orthodontic corrections — retreatment after prior orthodontic care, minor crowding or spacing, shifted teeth from not wearing retainers, or small cosmetic adjustments.

Participation requires an initial consultation and approval by a Forever Aligned Club Orthodontic Partner.

Treatment Process

Once enrolled and payment succeeds, you’ll receive two sets of aligners each month (upper and lower). Each aligner is worn for about two weeks before moving to the next set.

Treatment continues month-to-month until you’re satisfied with your results. Your first aligners are delivered at a scheduled office visit; future sets ship monthly after payment.

Enrollment & Program Fees

New enrollment: $250 enrollment fee + $250 first month’s payment ($500 total at start).

Ongoing monthly payment: $250/month.

Returning patients may resume treatment for $250/month with no new enrollment fee, as long as it’s within your original 5-year enrollment window.

Billing Authorization

By enrolling, you authorize Forever Aligned Club to automatically charge your payment method on file each month, and you agree to keep your billing information current.

Failed Payment Policy

If a payment fails, your next aligner shipment may be held until the balance is resolved. You’ll have 7 days to update your payment method before treatment is placed on hold.

Cancellation Policy

You may cancel the Smile Flex Plan at any time, with 30 days’ notice prior to your next billing date, after completing the required 4-month treatment period. Aligners already produced or shipped are non-refundable.

Program Duration

Your enrollment stays active for five years from your original enrollment date. You may start, pause, and resume treatment during this window without paying a new enrollment fee. After five years, new enrollment and scans are required.

Lost or Damaged Aligners

Lost or damaged aligners can be replaced for $15 per tray. Replacement production time may affect your overall treatment timeline.

Remote Monitoring

Forever Aligned Club may use Grin Scope remote monitoring technology to track your treatment progress. You may be asked to submit periodic scans or photos during treatment.

Retainers After Treatment

Retainers are not included with the Smile Flex Plan, but are strongly recommended to maintain your results. Retainers are available for purchase separately through Forever Aligned Club.

Orthodontic Supervision

The Smile Flex Plan is intended only for minor orthodontic movement. Forever Aligned Club reserves the right to discontinue this program if your case requires comprehensive treatment.
