Braces Treatment Program Terms & Conditions

Please read every section below carefully. This agreement outlines what to expect from your Braces treatment — from enrollment through your final adjustment visit.

Before you begin: This agreement covers your enrollment, billing, and treatment terms for the Braces Treatment Program. By signing below, you confirm that you’ve read and understood each section.

Program Description

The Braces Treatment Plan is a comprehensive orthodontic program designed to correct a wide range of bite and alignment issues — including moderate to severe crowding or spacing, bite correction, jaw alignment, and cases requiring precise tooth movement that removable appliances can’t achieve. Participation requires an initial consultation, diagnostic records, and approval by a Forever Aligned Club Orthodontic Partner.

Treatment Process

Once enrolled and payment succeeds, your braces (brackets and wires) will be placed at a scheduled office visit. You’ll return for adjustment appointments approximately every 6-8 weeks, during which your orthodontist will tighten or replace wires and make any needed changes to keep treatment on track. Treatment continues according to your personalized treatment plan until your orthodontist determines your case is complete and your braces are ready to be removed.

Enrollment & Program Fees

New enrollment: $250 enrollment fee + $250 first month’s payment ($500 total at start). Ongoing monthly payment: $250/month.

Billing Authorization

By enrolling, you authorize Forever Aligned Club to automatically charge your payment method on file each month, and you agree to keep your billing information current.

Failed Payment Policy

If a payment fails, your next scheduled adjustment appointment may be held until the balance is resolved. You’ll have 7 days to update your payment method before treatment is placed on hold.

Cancellation Policy

You may cancel the Braces Treatment Plan at any time, with 30 days’ notice prior to your next billing date, after completing the required 4-month treatment period. Braces already placed and any appliances already produced are non-refundable.

Program Duration

Your enrollment stays active for five years from your original enrollment date. You may start, pause, and resume treatment during this window without paying a new enrollment fee. After five years, new enrollment and diagnostic records are required.

Broken or Damaged Appliances

Broken brackets, loose bands, or damaged wires can be repaired for $15 per repair visit. Repeated breakage may extend your overall treatment timeline and, per our Early Termination Policy below, excessive breakage may affect your continued eligibility for care.

Remote Monitoring

Forever Aligned Club may use Grin Scope remote monitoring technology to track your treatment progress between office visits. You may be asked to submit periodic scans or photos during treatment.

Retainers After Treatment

Retainers are not included with the Braces Treatment Plan, but are required to maintain your results after your braces are removed. Retainers are available for purchase separately through Forever Aligned Club.

Orthodontic Supervision

The Braces Treatment Plan requires regular in-office adjustment visits under the supervision of your orthodontist. Forever Aligned Club reserves the right to modify your treatment plan if your case requires additional appliances or a different course of treatment.

Early Termination Policy

It is understood that Forever Aligned Club reserves the right to unilaterally terminate the doctor-patient relationship at any time, and for any of the following reasons, solely at the discretion of Forever Aligned Club:
  1. Failure to explicitly follow treatment recommendations and instructions.
  2. Poor oral hygiene is deemed detrimental to continuation of care.
  3. Excessively missed or rescheduled appointments.
  4. Excessive breakage of orthodontic appliances.
  5. Failure to keep financial commitments as stated in the Ortho Paid contract and related documents.
  6. Inappropriate behavior towards staff, doctors, or other patients, including negative posts on social media. Such posts will be deemed premeditated defamation, and legal action can/will be taken to maintain the reputation of Forever Aligned Club.
  7. Inability to achieve or maintain positive rapport with staff and orthodontist/dentist.
If Forever Aligned Club deems that the bilateral trust and respect necessary to support the doctor-patient relationship no longer exists, Forever Aligned Club will recommend another office for continuation of the patient’s care. The patient will be advised to seek continuation of treatment elsewhere, and all medical records will be transferred to that new office. Forever Aligned Club will no longer be deemed the treating office or office of record. By signing this document, the patient understands that Forever Aligned Club is not abandoning the patient, but rather finding the patient a new orthodontic home to better suit the needs of both the doctor and patient. Forever Aligned Club will be held harmless should the doctor-patient relationship be dissolved in this manner.
